These are commonly used forms and documents for members. They can help you manage your Blue Cross and Blue Shield of Texas plan. Many are PDF files. You may need to install a PDF reader program. Most PDF readers are a free download. One option is Adobe® Reader® which has a built-in reader. You can download this at Adobe's site.

Do you need a printed copy of your Member Handbook or Provider Directory? Call us and we will mail it to you at no cost. You can also request your plan document(s) in another language or format. You will receive them within 5 business days.

If you have questions about your plan, we can help. To ask about what the plan covers, find a provider, change your PCP and more, just call the Customer Advocate Department. We are available Monday through Friday, 8 a.m. to 5 p.m. Central Time.

If you call after hours, please leave a message. We’ll call you back on the next business day. Members with hearing or speech loss can call the TTY/TDD line at 711.

The machine-readable files have been compressed and provided in a ZIP format. Click on a link above to download and save a ZIP file to your computer. To unzip the file: Click on the ZIP file and you will have an option to unzip or extract. Follow the instructions. The process will create a new folder with the contents of the ZIP file (the full machine-readable file). Depending on your operating system, you may need to download a tool to open (un-zip) the file. There are tools available on the internet; some are at no cost.
