An In-Home Assessment involves:
- Clothed physical exam, including vital signs
- Review of medical conditions and medications
- Mobility and fall risk assessment
- Personal history, including drug, tobacco and alcohol screenings
- Diabetes and cancer assessments
- Depression, suicide and mental health screenings
- Clinician assessment with recommendations
- Preventive and chronic disease care recommendations
- Local resource suggestions
- Case management referral (if needed)
