Many Blue Cross Medicare Advantage (PPO) SM and Blue Cross Medicare Advantage (HMO) SM members haven't completed their annual health assessment (AHA). It is important to encourage these members to set up an in-person or telehealth AHA. Wellness visits provide opportunities to screen for new problems and manage chronic ones.
Incentive Payment
For every eligible AHA you complete between Jan. 1, 2021 through Dec. 31, 2021, you'll receive an incentive payment of $100 per Blue Cross Medicare Advantage (PPO) and Blue Cross Medicare Advantage (HMO) member, in addition to your contracted rate*.
Resources to get you started:
- Indices, a reporting platform, helps you identify which members haven't completed an AHA. Email your Network Management Consultant with your full name and NPI to get started with Indices.
- As a reminder, if during a wellness visit, you perform additional screenings or address another medical problem, you can submit an additional claim. To remove barriers to completion, several preventive services and certain wellness visits can be completed via telehealth in the member's home or a doctor's office.
