Have you submitted a Provider Onboarding Form to add a new provider, apply for credentialing or get contracted? Or recently submitted a Demographic Change Form to update your existing demographic information on our website? If so, you can now get online/real-time information about the status of these requests. In addition, you can also check the status of an email that was submitted to Network Management Offices.
Case Status Checker
When initiating any of the above processes, you will receive an email containing a unique case number which can be entered in the Case Status Checker.
The Case Status Checker provides a status of:
- Provider Onboarding Forms
- Updates to your profile information using our Demographic Change Form
- General email inquiries submitted for which you received a unique case number email after submission
Credentialing Status Checker
The Credentialing Status Checker provides information on the progress of your credentialing application by entering your National Provider Identifier (NPI) or state license number, where indicated, in the Credentialing Status Checker.
