2019 Notice to Physicians and Providers: Blue Cross and Blue Shield of Texas, a Division of Health Care Service Corporation (Blue Essentials^SM Blue Premier^SM, Blue Advantage HMO^SM, Medicaid (STAR), CHIP and STAR Kids) administers individual practice association health maintenance organizations that arrange for the provision of both basic and supplemental health services to their members. Any physician or provider may make application to join each of our networks; to be a participating physician or provider during the following 20 day period from Jan. 12, 2019, until Jan. 31, 2019.

Note: Only Physicians and Providers in the Fort Worth Star Telegram newspaper distribution area, you may make application during the following 20 day period from Jan. 14, 2019, until Feb. 2, 2019.
