New Member Welcome

After you or your child becomes eligible for CHIP and enrolls in the Blue Cross and Blue Shield of Texas (BCBSTX) CHIP Plan, we will send you some materials in the mail. Be on the lookout for the following and review the information you are given:

ID Card

After you are enrolled, we will give you your BCBSTX ID card. Keep this ID card with you at all times.

When you go to your doctor, hospital or other provider, show your BCBSTX ID card. It has the following information for each CHIP member:

  • Your name
  • Your member ID number
  • Your Medicaid number
  • The date your coverage starts
  • Your PCP's name and phone number

Call the Customer Advocate Department at 1-888-657-6061 (TTY: 711) to replace your ID card if it is lost.

The Health Assessment Questionnaire

If you are a new member, we will call you to do a Health Assessment over the phone. The assessment will give us extra information about your health so that we can send you health tips or tell you about programs we offer to keep you healthy. If you have not received a call to complete a health assessment within 90 days of joining the plan, call Service Coordination. They can help you complete an assessment.

If you are a current member and you have health concerns, call Service Coordination to get a health assessment. We have health programs for some chronic conditions that you can easily opt-in or opt-out of. Welcome Kit

After you get your ID card, you’ll get your welcome kit. It includes your Member Handbook. You’ll also get brochures on our transportation services and our value-added services. These handbooks tell you everything you’ll need to know about being a member of the BCBSTX CHIP program.

If you do not receive a welcome kit, please call the Customer Advocate Department at 1-888-657-6061 (TTY: 711). We’ll send out a replacement. We can also provide your kit in another language other than English or another format, such as Braille, CD or large print. We will send you this information in your preferred format for free.
