After you or your child becomes eligible for CHIP and enrolls in the Blue Cross and Blue Shield of Texas (BCBSTX) CHIP Plan, we will send you some materials in the mail. Be on the lookout for the following and review the information you are given:
After you are enrolled, we will give you your BCBSTX ID card. Keep this ID card with you at all times.
When you go to your doctor, hospital or other provider, show your BCBSTX ID card. It has the following information for each CHIP member:
Call customer service at 1-888-657-6061(TTY 7-1-1) to replace your ID card if it is lost.
You will also receive the Health Assessment Questionnaire. That helps us know what extra information we can send you to help keep you or your child healthy. Please take a few minutes to fill out the questionnaire and send it back to us in the self-addressed stamped envelope provided to you.

After you get your ID card, you’ll get your welcome kit. It includes your Member Handbook. You’ll also get brochures on our transportation services and our value-added services. These handbooks tell you everything you’ll need to know about being a member of the BCBSTX CHIP program.
If you do not receive a welcome kit, please call customer service at 1-888-657-6061 (TTY 7-1-1). We’ll send out a replacement. We can also provide your kit in another language other than English or another format, such as Braille, CD or large print. We will send you this information in your preferred format for free.
