We’re working with Esperta Health to offer in-home wound care services at no additional charge to certain commercial members who have chronic wounds.
What this means for you: Esperta Health customer care may reach out to you or your staff to confirm appointments or gather information to identify members eligible for this service. We encourage you to discuss this new offering with your qualified patients.
What does Esperta Health do? Esperta Health is an in-home specialty physician practice specializing in advanced wound care for chronic and at-risk wound patients. Board-certified wound specialists diagnose, treat and heal patients in the comfort of their home. Patients don’t need to be homebound to receive these services, and there are no orders for you to sign. Esperta Health returns patient visit summaries to you after the first visit, every 30 days and at patient discharge.

*Patients with at-risk wounds may have multiple or significant patient co-morbidities, recurrent infections, prior amputation(s), history of wound-related hospitalization, poor nutrition or a weakened immune system, including a chronic illness such as cancer, diabetes, chronic obstructive pulmonary disease or taking immunosuppressant medications, including steroids and biologics.
