If you are at risk for or already have a chronic condition such as asthma, diabetes or heart disease, our Condition Management specialists are ready to work with you and your doctor to help teach, coach and monitor your health.
If you are in the hospital, or recently visited the emergency room, a Care Management specialist may call to help setup any special care you might need as you recover. They will work with you to help you understand your condition and treatment options. These specialists will also help you get the care your doctor recommends and better understand your health care benefits, if needed.

Through a single point of contact, you and your doctor can get information about what’s covered under your plan and learn about other health care resources.
