Transition of Care

When you enroll in a new health plan, your provider network may change. If you are currently receiving treatment from a doctor who is not part of your new network, you may apply to continue treatment with that doctor temporarily. Review the following transitional care criteria for eligibility.

Examples of medical conditions that may meet the guidelines:

  • Pregnancy in the third trimester
  • Patients who are hospitalized prior to October 1st of the current year
  • Terminal illness where life expectancy is less than six months
  • Long-term treatment of cancer, heart disease, transplants

Examples of medical conditions that do not meet the guidelines:

  • Routine care (immunizations, physicals)
  • Chronic conditions that are stabilized (asthma, diabetes, heart disease)
  • Minor illness or injury (flu, sore throat, sprains)
  • Elective surgery

Starting July 6, 2026, contact a Health Advocate at 1-877-208-5306 if you (or a covered dependent) believe you are eligible for transitional care.

Blue Access for MembersSM

Your online account puts everything you need to know about your medical health plan right at your fingertips 24/7. You can sign up or log in to access detailed information about your account, coverage, benefits and claims. 

With your online account, you can:

  • Check the status of your claim and claim history.
  • Confirm who’s covered under your plan.
  • View and print an Explanation of Benefits for claims.
  • Find a doctor or hospital in your plan’s network.
  • Set your email and text alert preferences.
  • Request a new or replacement ID card or print a temporary one.

Go Mobile With the BCBSTX App

Keep your coverage information in the palm of your hand. With the BCBSTX Mobile App, you can:

  • View your Explanation of Benefits.
  • Access and download your digital ID card. 
  • Find in-network doctors, hospitals and urgent-care facilities.
  • Review your claims, coverage, deductible and out-of-pocket info.
  • Know your medical and prescription copays.
  • Take control of your wellbeing with preventive care guidelines, information and health tips for managing health conditions and living a healthier life.

The BCBSTX App is available in English and Spanish for iPhone and Android users. Download the BCBSTX App from the Apple App Store or Google Play.

BCBSTX ID Card

Learn about your BCBSTX ID Card:

  • ID cards show only the subscriber’s name, but they can be used by all dependents enrolled under the policy. 
  • Your Social Security number is not displayed on the card. Each card features a unique identification number that is listed on the card.
  • One ID card is issued for Single/Subscriber only policies. For Subscriber plus Spouse or Subscriber plus Spouse and children, etc. two cards are issued. 
  • Additional ID cards can be requested from your Health Advocate.
  • You can also download and print a copy of your digital ID card by logging in to your online account.
