What’s Changed
Please use the Provider Data Management module in Availity®or our Demographic Change Form to update your demographic information or to attest to your data. This enables us to meet the two-day provider update requirement defined under the Consolidated Appropriations Act.
Rejected Updates
We will no longer accept provider demographic updates via email, phone, or fax. Any demographic updates requested through these channels will be rejected and closed. Facilities should continue to use the Demographic Change Form. Learn more about updating your information.
Provider Onboarding Process
To apply to join our networks or add a provider to your current group, individuals and medical groups/clinics should use the Provider Onboarding Form. Learn more about applying to join our networks, including information for facilities.
