New Addresses for Submitting Payments for Claim Refund Requests

Beginning Oct. 1, 2019, we will be implementing new remittance addresses for claim refund returns. The new remittance addresses will be listed on the remittance form you receive in the mail with claim refund requests.

The fastest and easiest way to submit claim refunds for our commercial members is by using the Electronic Refund Management (ERM) tool. Refer to the provider website for information on using ERM. For government members or if you do not wish to use the ERM tool for commercial members, providers should avoid delay by using the new addresses below to mail claim refunds.

Claim refund returns received at our old addresses will be forwarded for a minimum of 90 days. After the forwarding service ends, any payments submitted to the old addresses will simply be returned to the sender.
