Revalidation Application Required to Receive a Second or Third Extension

Feb. 9, 2026 (Updated Feb. 19, 2026)

What’s new: On Feb. 1, 2026, the Texas Health and Human Services Commission implemented additional eligibility criteria for second and third revalidation due date extensions. There were no changes to first-time extension criteria. Blue Cross and Blue Shield of Texas must verify extension approval by reviewing the Medicaid Provider File.

Second and third due date extension eligibility criteria: Second and third due date extensions aren’t automatic. Eligibility criteria must be met, including revalidation application submission. See the Texas Medicaid & Healthcare Partnership notice for details. A summary is below for quick reference.

Why it matters: Revalidation ensures enrollment information in your existing Medicaid Provider File is up to date. If you don’t meet revalidation process deadlines, you may be disenrolled from Texas state health care programs. This could lead to prior authorization request and claim denials.

Second Revalidation Due Date Extensions

As of Feb. 1, 2026, providers are eligible for a second revalidation due date extension of 180 calendar days only if both these criteria are met:

  • The provider’s current revalidation due date is on or before May 31, 2026, based on having already received one 180-day extension; and
  • The provider has submitted a revalidation application in the Provider Enrollment and Management System prior to their revalidation due date.

Third Revalidation Due Date Extensions

As of Feb. 1, 2026, providers are eligible for a third revalidation due date extension of 60 calendar days only if both these criteria are met:

  • The provider’s current revalidation due date is on or before May 31, 2026, based on having already received two prior 180-day extensions; and
  • The provider has submitted a revalidation application in PEMS prior to their revalidation due date.
