Reminder
As a reminder to our Blue Cross and Blue Shield of Texas (BCBSTX) Medicaid providers, to receive reimbursement for Sport/Camp Physicals, please do not submit the Sports/Camp physicals through the claims system, as this service is not a benefit of Texas Medicaid.
Follow these steps:
- Primary Care Provider (PCP) should verify the eligibility of assigned member and confirm there has not been a physical within one year of the last physical.
- Providers must conduct a physical that meets the minimum requirements defined by a Sports or Camp Physical.
- Provider must submit a Sports and Camp Physical Reimbursement Form to BCBSTX within 95 days of the date of services. The form must be filled out and mailed to:
