Our Blue Cross and Blue Shield of Texas (BCBSTX) Disease Management program for our STAR, CHIP, and STAR Kids plans are based on a system of coordinated care management interventions and communications designed to assist physicians and others in managing members with chronic conditions.
The Service Management/Service Coordination team is staffed by individuals with extensive medical and behavioral health experience in such areas as asthma, bipolar disorder, heart failure, diabetes, and hypertension.
Early intervention is essential to maximizing treatment options while minimizing potential complications associated with catastrophic illnesses, injury, and exacerbation of chronic conditions. Our Service Management/Service Coordination (Disease Management) team is available Monday through Friday from 8 a.m. to 5 p.m. (CST) to assist with the coordination of our member’s needs.
