Update: Kate Farms Voluntary Formula Recall Expanded

On August 10, 2022, Lyons Magnus (“Lyons”), LLC expanded the voluntary recall initially announced on July 28, 2022, to include the following Kate Farms products. Several of Lyons Magnus products were recalled because of possible microbial contamination. Our members may have been affected by this recall and have questions about the contaminated products.

This expanded recall also impacts many other products manufactured by Lyons Magnus that are not available through WIC. 

Lot numbers for Kate formulas listed:

  • KARMS Pediatric Peptide 1.5 Ready to use (RTU) 8.45 oz
  • KFARMS Pediatric Standard 1.2 RTU 8.45 oz
  • KARMS Peptide 1.5 RTU 11 oz
  • KFARMS Standard 1.0 RTU 11oz
  • KF Pediatric Peptide 1.0 RTU 8.45 oz

Lot numbers for Pediasure Harvest and Ensure Harvest listed:

  • Pediasure Harvest RTU 8 oz
  • Ensure Harvest RTU 8 oz

*The Lot Number can be located on the top of the packaging, below the date of the product.

In response to this recall, Blue Cross and Blue Shield of Texas (BCBSTX) will ensure our members have access to covered services, education on the availability of non-capitated services, and appropriate referrals.

Our members can find additional information and available resources on Texas WIC website. There may be an urgent need to ensure that certain members have access to formula if their current formula is involved in the recall.
