As of April 1, 2026, Memorial Hermann doctors, health care professionals and facilities are no longer part of our Blue Choice PPO^SM, Blue Essentials^SM, Blue Advantage HMO^SM and BlueHPN^®  provider networks. 

In-network providers: We’ve contracted separately with some of the community doctors and health care professionals affiliated with Memorial Hermann. This will allow you to continue seeing these doctors at in-network rates. We are updating our online directory with the latest status. In the meantime, check the list below for those who are now out of network.
