Designed to help our members manage and improve their health, Blue Care Connection provides a total care approach for members interested in wellness and prevention or for those who need more complex case management. To ensure members get the most from the services offered through Blue Care Connection, we have developed a variety of communications that employers may provide to their employees. You can download artwork or text files - in English or Spanish - that is ready to print and distribute, or to use electronically.
The Personal Health Manager (PHM) is one of the resources available to groups with the Blue Care Connection program and offers many personalized online resources to maintain a healthy lifestyle. Employers can assist members in learning more about the Personal Health Manager through Flash presentations that can be viewed online or downloaded for posting on an intranet or Internet Web site. To access the Personal Health Manager, log into Blue Access® for Members and click on the Personal Health Manager icon located on the home page.
