Blue Cross and Blue Shield of Texas (BCBSTX) incorporates the following Clinical Practice Guidelines (CPGs) into its Condition Management Programs. BCBSTX updates CPGs at least every 2 years but will review them more frequently, if necessary. You may send questions or comments about these CPGs to ClinicalPracticeGuidelines@bcbstx.com

The Blue Review provider newsletter features articles regarding the availability of the nationally recognized CPGs as well as the BCBSTX Condition Management Programs.  The Condition Management Programs offered by BCBSTX include advanced care planning, asthma, chronic/congestive heart failure, chronic obstructive pulmonary disease, chronic pain, coronary artery disease,  depression in primary care, diabetes mellitus, geriatrics, hypertension, metabolic syndrome, tobacco cessation and weight management.  These programs are offered by BCBSTX to selected HMO's, all Fully Insured members and  Blue Choice PPO members whose employer group has purchased the programs.  If you have a patient who you feel would benefit from one of the above mentioned programs, please call 866-412-8795 to enroll them today.
