Care Management Program

UT CONNECT provides voluntary holistic care management services designed specifically for those living with chronic conditions or dealing with complex health care needs. When you enroll in the program, you'll receive helpful information about your condition, at no out-of-pocket cost to you. The holistic care management program works together with you and your family, your health plan, and doctor to help identify the best way to manage your condition more effectively.

Chronic conditions such as asthma, diabetes, congestive heart failure, coronary artery disease, metabolic syndrome (high blood pressure, high cholesterol), lower back pain or end stage renal disease can lead to decreased quality of life and high medical costs when they are not well managed.  You may also be dealing with severe illnesses or injuries resulting in more complex needs and long term hospital confinements. You will have a registered nurse available to offer individualized support in managing these conditions and their associated costs. The assigned nurse will answer questions, explain options, and identify your needs in order to help you and your family receive the most appropriate, cost effective care.
