Blue Cross and Blue Shield of Texas (BCBSTX) offers certain services for members with special needs. These services are available at no cost and include Service Management support. We can help you find the right level of service management support, which may include providing you with a Service Manager.
A Service Manager will work with you and others involved in your care, like your primary care physician (PCP), to help with your health care needs. Your Service Manager will also work with you to make a Care Plan that helps you reach your health goals.
When you choose BCBSTX for STAR, you have access to programs and classes that can help you live healthier.
Our health care programs will help you take better care of yourself if you have:
- Asthma
- Diabetes
- Heart Disease
Service Managers also do the following:
- Plan in-person visits or phone calls with you
- Listen to your concerns
- Help you to get the services you need, like transportation
- Help set up care with your doctor and other health care team members
- Help you, your family and your caregiver better understand your health condition(s), medications and treatments
