  • Texas residents ages 60 days to 65 years old
  • Non-expectant parents
  • Unmarried children ages 60 days old and under 25 years old
  • U.S. citizens or non-U.S. citizens living in the United States for at least two years (a copy of your Alien Registration Receipt Card must be sent in when you apply)
