PPO Select Choice Series III offers you many choices that help you control the monthly cost of your insurance protection and limit your out-of-pocket health care costs, including the choice of deductibles and your choice of health care providers. With eight plan options to choose from, PPO Select Choice Series III is flexible enough to meet your individual needs, family situation, and your budget. Premiums are based on plan option, age, sex, geographic location and number of family members to be covered. The higher deductible you choose, the lower your premiums can be! Initial rates are guaranteed for six months except for entry into a premium age bracket due to a birthday or a change in residence that results in a different rate.
