On 10/8/2018, Blue Cross and Blue Shield of Texas (BCBSTX) determined there were several codes missing from the 1/1/2019 Prior Authorization List published on 09/17/2018. A revised list has been posted to the BCBSTX provider website under Clinical Resources. Select Prior Authorization & Predeterminations. Then scroll to and select Blue Cross Medicare Advantage PPOSM and Blue Cross Medicare Advantage HMOSM to view the current listings.
