Effective November 1, 2012, the maximum allowable fee for CPT code 55250, when performed in a physician’s office, will be the result of multiplying the relative value (RV) posted on the BCBSTX web site under the "BCBSTX Relative Value" section, and the applicable Surgery conversion factor.
