- Two group numbers: 038000 and 038001
- ID cards do not expire annually and are not reissued each year
- Benefits do not change on January 1 or at the first of each month. If benefits change, it will occur on September 1
- Out-of-Pocket Maximums are calculated on a calendar year (January 1 – December 31) rather than the plan year (September 1 – August 31)
- No out-of-pocket maximum amounts carry over to the following year
