One of our large national employer groups, Nissan North America, has implemented a health (biometric) screening program that will be administered by StayWell, a health management company. Nissan employees who participate are provided 100% coverage for certain health screenings.

Click on the links provided below to view the information that will be included in a packet that has been mailed to approximately 1,250 Nissan employees. We are making you aware of this action in the event that a Nissan employee presents the form and letter to a participating BlueChoice PPO provider. If the participating BlueChoice PPO provider administers the health screening and completes the StayWell Health Care Provider Form on behalf of the Nissan employee, the provider will be reimbursed 100% of the allowable amount for the screenings indicated in the packet. The provider will not need to collect a copayment or coinsurance since the benefit will be paid at 100% of the allowable amount.
