Select Blue Advantage Series III, a Preferred Provider Health Insurance Plan, emphasizes preventive care with coverage for routine physicals, immunizations and diagnostic tests. When network providers are used for medical care, office visits and certain office setting services are generally covered in full, subject to the copay. So, policyholders could receive the services outlined below for $25 per visit! Some services, such as immunizations for children through age seven (7), are covered at 100 percent, of the allowable amount, no copayment required. Immunizations for age eight (8) and over are paid at 100 percent of allowable amount after copay up to $300 calendar year maximum per participant.
