Preventive Care

Preferred Provider Health Insurance Plan (PPO) Select Basic^SM emphasizes preventive care with coverage for routine physicals, immunizations and diagnostic tests. When policyholders use network providers for their medical care, office visits and certain office setting services are generally covered at 75 percent of the allowable amount, subject to deductible and coinsurance. Some services, such as immunizations for children through age seven (7), are covered at 100 percent of the allowable amount.
