Preferred Provider Health Insurance Plan (PPO) Select Basic offers you four (4) options that can work for each situation and budget. And since there is a choice of deductibles, policyholders can adjust their monthly premium. They can choose a higher deductible to lower their monthly premium. Policyholders can control their costs more when they choose providers or hospitals from our BlueChoice® network.

Premiums are based on health insurance plan options, age, sex, geographic location, tobacco use, number of family members to be covered and evaluation of health. Initial rates are guaranteed for six months, unless a policyholder enters a new age bracket due to a birthday, or a change in residence results in a different rate.
