PPO Select Value Care, a Preferred Provider Health Insurance Plan, emphasizes preventive care with coverage for routine physicals, immunizations and diagnostic tests. When you use network providers for your medical care, office visits and certain office setting services are covered at 50 percent of the allowable amount. Some services, such as immunizations for children through age seven (7), are covered at 100 percent of the allowable amount.
