Preferred Provider Health Insurance Plan (PPO) Select Value Care is flexible enough to meet your individual needs, family situation, and your budget.

Premiums are based on health insurance plan option, age, sex, geographic location, tobacco use, number of family members to be covered and evaluation of health. The higher coinsurance maximum level you choose, the lower our premiums can be! Initial rates are guaranteed for six months except for entry into a new premium age bracket due to a birthday or a change in residence that results in a different rate.
