PPO Select^® Choice Series III, a Preferred Provider Health Insurance Plan, emphasizes preventive care with coverage for routine physicals, immunizations and diagnostic tests. When you use network providers for your medical care, office visits and certain office setting services are generally covered in full up to certain dollar maximums after your copay. So, depending on the plan you choose, you will receive the services below for $25 per visit! Some services, such as immunizations for children through age seven (7), are covered at 100 percent of the allowable amount.
