Preferred Provider Health Insurance Plan (PPO) Select Choice Series III offers you many choices that help you control the monthly cost of your insurance protection and limit your out-of-pocket health care costs, including the choice of deductibles and your choice of health care providers. With eight (8) plan options to choose from, PPO Select Choice Series III is flexible enough to meet your individual needs, family situation and budget.

Premiums are based on health insurance plan option, age, sex, geographic location, tobacco use, number of family members to be covered and evaluation of health. The higher deductible you choose, the lower your premiums can be! Initial rates are guaranteed for six months except for entry into a premium age bracket due to a birthday or a change in residence that results in a different rate.
