PPO Select Basic^SM is a Preferred Provider Insurance Plan (PPO) and emphasizes preventive care with coverage for routine physicals, immunizations and diagnostic tests. When you use network providers for your medical care, office visits and certain office setting services are generally covered at 75 percent of allowable amount, subject to deductible and coinsurance. Some services, such as immunizations for children through age seven (7), are covered the 100 percent of the allowable amount.
