Preferred Provider Health Insurance Plan (PPO) Select Basic offers you four (4) plan options that work for your individual situation and budget. And since you have several deductibles to choose from, you can do even more to control the cost of your monthly health insurance premium. Simply choose a higher deductible and you can lower your monthly premium. Your initial rates are guaranteed for six months, unless a customer enters a new age bracket due to a birthday, or a change in residence results in a different rate. And as a PPO Select Basic plan customer, you can help control your health care costs even further when you select physicians or hospitals from our expansive BlueChoice network. Premiums are based on insurance plan options, age, sex, geographic location, tobacco use, number of family members to be covered and evaluation of health.
