PPO Select BasicSM, a Preferred Provider Health Insurance Plan, offers you and your family:
- $5,000,000 lifetime maximum benefit per person

- Well-child care, routine physical, vision and hearing (subject to deductible)
- Immunization benefits (through age seven)
- Human organ and tissue transplant benefits ($300,000 lifetime maximum, subject to $5,000,000 policy maximum).
- Office visits and emergency room visits subject to deductible and coinsurance
- Home health care, hospice and skilled nursing benefits (subject to limitations)
- Age-banded rates for child-only coverage
- Access to one of the largest provider networks in the state, BlueChoice®
- Security of one of the most widely recognized insurance cards – Blue Cross and Blue Shield of Texas
- Blue Cross and Blue Shield of Texas Three-tier pricing prescription drug plan

- Preventive Care – Up to maximum of $300 per person per calendar year, subject to deductible and coinsurance. This added feature allows policyholders to receive negotiated provider discounts (when using In-Network providers) for allowable preventive care services up to the $300 annual maximum.
