• Choice of inpatient providers and hospitals (out-of-network covered at a lower level of benefit)
• Lifetime maximum coverage of up to $2 million per person after deductible, copay and coinsurance
• Calendar year deductible of $5,000 individual and $15,000 family for in-network services
• Calendar year out-of-pocket maximums of up to $5,000 individual and $15,000 family for in-network services
• Emergency care coinsurance of 20 percent of the allowable amount after deductible and copay (in-network and out-of-network hospital charges within 48 hours of admittance are covered; physician charges are not covered)
