When you use network providers for your medical care, office visits and certain office setting services are generally covered at 90 percent (for Plans I - III), 75 percent (for Plans IV - VI) and 100 percent (for Plan VII) of the allowable amount, subject to deductible. Some services, such as immunizations for children through age six (6), are covered at 100 percent of the allowable amount.
