Earn 5,000 Blue Points for taking the Health Risk Assessment*! You and your covered dependents can earn 5,000 annually for taking the Health Risk Assessment.
Get involved and take a Health Risk Assessment…it is easy to understand and takes approximately 10 to 15 minutes to complete. You can also complete the assessment several times throughout the year to track your progress.
Once you have completed the assessment, you will receive an individualized report that will provide you with guidance and suggestions on the next steps to improving your health.
Please be assured your information is kept confidential and will not be released to your employer or outside entities without your authorization.
*The points will be earned for completing the Health Risk Assessment every six months.

Log in to Blue Access for Members and click on the Personal Health Manager logo found on the home page of Blue Access for Members or under the My Health tab.
- Once in the Personal Health Manager, you'll see a section called "Know Your Risk" located in the top right hand corner of the screen. Within this section click on the link to complete a Health Risk Assessment.

If you are not a registered user with Blue Access for Members:- Have your subscriber number and group number ready - you can find this on your HealthSelect ID Card.
- Log in to Blue Access for Members.
- Create a user name and password for immediate, secure access to your personal information.
