"Medical Home" Pilot Launched in DFW Area
Effective February 1, 2010, BCBSTX and Medical Clinic of North Texas, P.A. are implementing a "Medical Home" pilot. The "Medical Home" is a patient-centered health care delivery model that has gained national attention.
The "Medical Home" is not a building, a house, or a hospital. It is a primary care physician based concept designed to improve coordination of care within the medical care system with emphasis on wellness, prevention and disease management.
