Disease and Condition Management Programs are designed for those who have been diagnosed with conditions such as asthma, diabetes, congestive heart failure or coronary artery disease, to name a few.
When you are selected to enroll in one of our programs, you will be contacted by a BCBSTX Nurse, a Blue Care Advisor, by either phone or mail. This Blue Care Advisor will be your personal telephone coach who works with you and your physicians in managing your condition more effectively.
Your Blue Care Advisor will help you coordinate your health care and benefits; educate and empower you to make informed choices and promote wellness by encouraging self-management.
