As a HealthSelect participant, you have access to Disease Management Programs through Blue Care® Connection. Disease Management Programs* are designed for those participants who have been diagnosed with conditions such as asthma, diabetes, congestive heart failure, or coronary artery disease, to name a few.
Participants who may benefit from this type of program may be contacted by a Blue Care Advisor (a BCBSTX Nurse), by either phone or mail inviting them to enroll. This Blue Care Advisor will be your personal telephone coach who will work with you and your physicians in managing your condition more effectively.
Your Blue Care Advisor will help you coordinate your health care and benefits, educate and empower you to make informed choices, and promote wellness by encouraging self-management.

Stop Smoking and Weight Loss Tools
If you need support and guidance to quit smoking or lose weight, BCBSTX offers both online and telephonic tobacco cessation and weight loss programs as part of your HealthSelect coverage.
While in the Personal Health Manager, you can enroll in a self-paced online tobacco cessation or weight loss program that allows you to track your progress, read articles, and ask questions. By using the online options, you will also be eligible for Blue Points!
If you are interested in a program that provides you a personal coach, you can enroll in a Lifestyle Management (tobacco cessation and weight loss) program by calling BCBSTX at (800) 462-3275. Once enrolled, you will be assigned a dedicated telephone coach who will set goals with you, track your progress, and help you on your journey to better health.

If you are a future HealthSelect participant, you will have access to Disease Management Programs offered through Blue Care Connection once your coverage goes into effect.
