When you take your Health Risk Assessment between December 3, 2008, and February 20, 2009 (the “Program Period”), Blue Cross and Blue Shield of Illinois*, Blue Cross and Blue Shield of New Mexico*, Blue Cross and Blue Shield of Oklahoma* and Blue Cross and Blue Shield of Texas*, will donate $5 to the American Heart Association, up to a maximum of $75,000. Eligibility: Donation eligibility is open only to legal residents, age 18 or older, who are current members or become members of Blue Cross and Blue Shield of Illinois, Blue Cross and Blue Shield of New Mexico, Blue Cross and Blue Shield of Oklahoma or Blue Cross and Blue Shield of Texas during the Program Period. Eligible members must register or be registered to use the Blue Access for Members Web site and complete their online Health Risk Assessment during the Program Period. Subscribers and subscriber-dependents who have already completed their online Health Risk Assessment are not eligible. Void where prohibited.
